AXIS is a physician-led post-acute transitional care and rehabilitation practice serving patients throughout the Las Vegas Valley. We partner with hospitals, skilled nursing facilities (SNFs), inpatient rehabilitation facilities (IRFs), long-term acute care hospitals (LTACHs), and community providers to ensure seamless transitions of care, improve functional outcomes, reduce avoidable readmissions, and support patients throughout their recovery. Through comprehensive Physical Medicine & Rehabilitation, Pain Management, and Supportive Care services, our goal is to help patients restore function, maximize independence, and improve quality of life.

Founder, Medical Director & Attending Physician

Carlos A. Rodriguez, DO





Dr. Carlos Rodriguez is a residency-trained Physical Medicine & Rehabilitation physician who is also fellowship-trained in Hospice and Palliative Medicine.

He specializes in post-acute transitional care, rehabilitation medicine, pain management, and supportive care, partnering with healthcare facilities throughout Southern Nevada to improve recovery, function, and quality of life.

Non-operative management of acute and chronic pain conditions with individualized treatment plans designed to improve comfort and daily function.


Supportive Care


Patient-centered care focused on symptom management, quality of life, goal setting, and support for patients and families facing serious or complex medical conditions.


Care Coordination & Facility Partnerships


Collaboration with hospitals, SNFs, IRFs, LTACHs, home health agencies, and community providers to ensure seamless transitions across the continuum of care.
